These are the documents you will have to provide for your Nigerian passport appointment date.
Please read carefully and provide the ones applicable to you.

Passport Renewal

Applies to both adults and minors (under 18s). Applicants must have previous passport booklet in their possession, not lost.

Current or Expired Nigeria passport booklet

Change of Name by Marriage

Applies to only to married women that want to take their husband’s Last name (Surname). Can be renewal or lost passport

  1. Marriage certificate -Original & Photocopy
  2. Letter written by husband giving consent to wife to change her last name (surname) to his. Letter must be signed (signature must match that shown in his passport). Letter can be written by hand
  3. Copy of husband’s passport must be provided.

First Issue Adult Passport

This applies to adult person(s) who are getting their Nigerian passport(s) for the first time and were not born in Nigeria

  1. Birth certificate containing names of parents only applies to those born outside of Nigeria – Original & Photocopy
  2. Copy of parent’s Nigeria passport (Bio-data page). Just one of the named parents in birth certificate should provide a passport it must be a Nigerian passport. If the parent is deceased, then a death certificate – Photocopy

First Issue Minor (under 18s) Passport

This applies to children (under 18s) who are applying for their Nigerian passport(s) for the first time and were not born in Nigeria

  1. Birth certificate containing names of parents (only applies to those born outside of Nigeria) – Original & Photocopy
  2. Copy of both parents passport (Bio-data page). One of the passports provided must be Nigerian.(This applies only those parents named in the birth certificate) – Photocopy
  3. Minor consent form filled and signed by both parent(s) whose names are written in the passport – (link to minor consent form can be found here – Minor consent letter
  4. Single parents must provide a court order giving them custody of the child. (*This only applies if the father’s or mother’s name appears on the birth certificate however he or she is an absent)
    In the situation that the one of the parent is deceased, a death certificate should be provided
  5. Copy of any other passport the child possesses if they have one
